- certificate of secondary education and its application (original and copy);
- certificates of the Ukrainian Center for Educational Quality Assessment (original and copy) of the Ukrainian language and literatute, mathematics and physics;
- passport and a copy of the first pages and pages of registration of residence;
- identification code (original and copy);
- medical certificate on Form 086;
- six photos 3×4;
- military card or registration certificate (for boys).
